Fax orders . . . Orders may be placed via fax. Please include item name, quantity, billing and shipping address, credit card information; to include credit card type, number, expiration date, CVV2 number (3 digit code on back of card signature bar), exact name as it appears on the card, billing address, phone number, and email address.
